Hearing loss and the world of the d/Deaf
First of all, many thanks to @thelouistiti​ for your question about resources on hearing loss. I decided it would be better to write everything myself rather than sharing a compilation of links.
Deafness/hearing loss is huge topic and it's two-fold: there's the medical aspect and the cultural one. When talking about Deaf culture and community, I'll capitalize the "d" as is customary to make the distinction. I'll talk from a French perspective, and as a person with severe acquired hearing loss, who is currently learning sign language and making her first steps into the Deaf community.
If you feel I'm too vague, that I need to amend things, or if you just would like to chime in and talk about your experience, my ask box is open and I'll share your input.
For the Frenchies and French speakers here, I also suggest this episode of "C'est pas sorcier" which is about the d/Deaf world. It's old as balls (late 90s) and many things have evolved since (FSL is now considered an official language in France), but they talk about the different types of hearing loss (conductive and sensorineural), how the ear works, hearings aids, and they tackle a bit Deaf History in France. It's a good introduction.
Now let’s begin if you’re ready!
What's hearing loss, how is it measured and what are the different thresholds?
Hearing loss generally refers to the loss of acuity in one or both ears. A person with hearing loss will have difficulties singling out sounds in a noisy environment, understanding speech and pinpointing where a sound originates, hearing a TV and making phone calls. There are a number of factors that can cause hearing loss such as: exposure to noise, ageing, genetics, medication, infections, ear trauma, etc.
Hearing loss is measured by the number of decibels necessary to perceive a sound on a scale of 0 to 8000 Hz (low to high frequencies). One will be considered hearing impaired when unable to perceive a sound under 25 dB. There are 4 thresholds, or degrees of hearing impairment: mild (25 to 40 dB), moderate (41 to 70 dB), severe (71 to 90 dB) and profound (greater than 90 dB). At 120 dB, the loss is considered total.
Hearing loss isn't necessary uniform across all frequencies: some people will have no trouble hearing low frequencies without being able to perceive anything higher-pitched than 1000 Hz.

Other than severity, hearing loss is also categorized in 3 main types: conductive, sensorineural and mixed (the latter being a combination of conductive and sensorineural).
Conductive hearing loss: the sound doesn’t reach the cochlea (the inner ear). The causes can be infections, a damaged eardrum, malformations, impacted earwax among others.
Sensorineural hearing loss: dysfunction of the inner ear. The sound is able to reach the cochlea, but isn't handled properly. The most common cause is the destruction of the hair cells in the cochlea.
There is another auditory disability that isn't exactly hearing loss but hinders one's ability to understand sounds: auditory processing disorder. People with APD have normal hearing and no damage to the inner ear, but the sound isn't processed properly by the brain.
I'm keeping this short, but the corresponding Wikipedia pages are quite exhaustive, with statistics and all.
How to tell if one is deaf or hard of hearing (HOH)?
Well, it's actually complicated and depends on the person. I've noticed everyone has their own definition of deaf and HOH. According to some people, only people with profound to total hearing loss should be called deaf. Medically, it seems that people with severe hearing loss can also be considered deaf. I've seen people say that you have to be born deaf to call yourself so. And I guess that among hearing people, many think deaf people are simply the ones  communicating in sign language.
And there's my personal take: you call yourself what you feel best describes your hearing abilities and problems. I feel that if hearing loss severely hinders your social and professional life, you may call yourself deaf. The degree of your loss doesn't matter that much if you feel isolated by your disability.
And to hearing people: you don't get to have a say if one is deaf or HOH. Period.
What are hearing aids?
There are actually 2 kinds of hearing devices: hearing aids and cochlear implants.
Hearing aids:
They improve hearing by making sounds audible, for example by amplification. Their settings are custom-made for the wearer by an audiologist. They come in all shapes and sizes but there are 2 main families: Behind the ear (BTE) and In the ear (ITE) hearing aids. The former are composed of a case attached to an earpiece that fits inside the ear. The case rests behind the ear and contains the electronics, controls, battery, and microphone, while the loudspeaker, or receiver, may be housed either in the case or in the earpiece These are particularly recommended for more severe hearing losses, but they're very versatile. In the ear hearing aids devices fit completely in the ear bowl (no case behind the ear) and are more discrete. They're usually recommended for mild to some severe hearing losses
Hearing aids can't completely correct hearing loss. Sensorineural hearing loss reduces the sensitivity to sound and aids can only partially compensate by amplifying sounds. Conductive hearing losses tend to be better treated by hearing aids; the amplified sound is able to reach the cochlea, and the signals are transmitted normally to the brain.
Cochlear implants:
You know how I said sensorineural hearing loss was generally caused by the destruction of hair cells inside the cochlea? These hair cells are sensory receptors that trigger nerve signals when detecting sound waves. The cochlea then converts those sound waves into electric signals that travel to the brain through the auditory nerve. So with damaged hair cells, the signals can't properly reach the brain and the information will be incomplete.
A cochlear implant bypasses the outer, middle and inner ear and generates the electric signals. It doesn't produce noise and unlike hearing aids, it doesn't amplify it. The outer part is composed of a microphone, a sound processor and a coil. The mic takes in sound, then passes it to the processor that crunches the data into electric signals called "channels", which correspond to different sound frequencies. The data is transmitted to the coil, placed on the scalp. The signals are then transmitted as wireless signals to a receiver and electrode array surgically implanted into the cochlea. The receiver converts the waves into electric impulses and when the signals reach the electrodes, they trigger the hearing nerve connected to the brain.

What an implanted person hears is what the brain interprets from the combination of these signals. The limited number of channels in a cochlear implant means the sounds are very different from natural hearing and it is necessary to teach the brain to hear after the surgery. It typically takes months of auditory training. The implant doesn't replace hair cells and can't relay pitch and timbre. Implantation is also irreversible. But it allows people with a very low level of hearing to hear speech and pick up different types of sound. When one doesn’t benefit enough from hearing aids, it’s often the last alternative.
It is believed that implanting congenitally deaf children at a young age gives better results, but this is a very controversial topic and I'll get back to it later.
In France, hearing aids are expensive and cochlear implants even more so, but partially reimbursed (fully for children) by social security.
Are deaf people mute?
It’s a common misconception. When you have hearing loss, your vocal cords aren’t impacted. Deaf people can speak, and some really well; they sometimes just choose not to because it can be difficult to regulate volume and pitch. Some deaf people get discriminated for their accent.
So please, ditch the word deaf mute (”sourd-muet” in French).
Can deaf people listen to music?
Yes, but not with the same accuracy. People with profound to total hearing loss will mostly feel the beat and vibrations. There are backpacks and vibrating floors specifically designed for this purpose. People with hearing aids can link them to their music devices with Bluetooth or just wear headphones. The only exception, I believe, is cochlear implants. As they can’t relay pitch and timbre, music isn’t perceived as accurately. It gets better once the ear is trained.
By the way, try to be mindful with deaf people about music and don't imply we miss out on something great. Some don't care, some are perfectly content with what little they can hear and feel. And those who lost their hearing later in life don't need reminding.
How do you manage in your daily life with hearing loss?
Firstly, I would like to stress that most of the issues faced by deaf and hard-of-hearing people are social, not medical, we don’t usually suffer physically from our condition.
Regarding adjustment, it's different for everyone. How the person copes depends on lots of factors, including early vs. late hearing loss, gradual vs. sudden, as well as the severity, the communication needs and the personality (acceptance vs. aversion to change). Hearing loss is also linked to feelings of depression, anxiety, frustration, social isolation and fatigue.
Hearing loss hinders access to education and job opportunities due to impaired communication. In France, the unemployment rate of people with severe hearing loss is almost 4 times the national average. There are extremely few universities which offer proper higher education in sign language. Much of the available higher education adapted to the deaf and hard-of-hearing is short-termed and still very oriented towards manual jobs. And it's not necessarily easier for mainstreamed oralist deaf/hard-of-hearing people who attend universities. As a consequence, deaf people have lower levels of education, with only 10% getting a higher education diploma compared to 30% of the general population.
Access to services, infrastructure and culture is also still a problem. A very important law was passed in France in 2005 in favor of people with disabilities. It guaranteed, among other things, access to all areas of social life: education, employment, housing, transportation and culture. But progress has been very slow. In France, in 2020 for instance, it's next to impossible to find French movies with French captions in cinemas. However, almost all foreign productions in theaters here are available in their original version with French subtitles. Close captions are subtitles specifically designed for the deaf and hard-of-hearing. They supplement the dialog with all the other audio cues: ringtones, music, languages, and so on. They're also more expensive and there are few sessions offering them. Regarding telecommunications, we had to wait until October 2018 for a national telecommunications relay service that allows us to make phone calls with communications operators (FSL interpreters) and speech recognition.
Social integration is extremely important to our well-being. A lack of accessibility may be the cause of a drop in self-esteem and confidence, as well as social withdrawal. Hearing loss has been linked to Alzheimer's disease and dementia, especially among isolated individuals. Prejudice and discrimination are also a problem, probably more towards signing deaf people because both “slurred” speech and the inability to speak are perceived as alien.
Look, lack of accessibility is a pain, but it really shouldn’t prevent us from living our lives to the fullest. The important thing is to not stay alone, even though it's sometimes easier said than done. There is a lot of systemic pressure to make hearing loss invisible. Invisibility is even a selling point for hearing aids, while implanting children at an early age is encouraged to raise them in the hearing world… Many HOH people don't get access to the community simply because they don't know it exists! 
Do you have to learn sign language when you become deaf?
No, you don't have to. The later you become deaf, the more you'll tend to continue using speech. But in my opinion, it's worth considering learning sign language for people with acquired hearing loss, especially children, even if they are fitted with hearing aids. Why? Because hearing aids can't do everything and their performance is very limited in noisy environments. It's great to have another ace up your sleeve to communicate. Also, and it's my personal experience, it's comforting to be in touch with the deaf community and people who share your joys and problems alike.
This is a very sensitive and controversial topic within the Deaf community, which is why I choose to give my opinion on acquired and not prelingual hearing loss. Prelingual hearing loss refers to people who were born deaf or became deaf in their infancy, before the acquisition of language. It means parents must choose how they will educate their children, if they will have them fitted with hearing aids or implanted, follow a mainstream education or go to a deaf school…
Deaf and hard-of-hearing people may also use cued speech (”Langue Parlée Complétée” or “LPC” in French). Cued speech is a visual rendering of spoken words. Some phonemes like "t" and "d" or "p" and "b" can't be differentiated when reading lips (which deaf and HOH people often do automatically). Cued speech renders all phonemes visible through handshapes, known as cues. They are 8 handshapes to represent consonants and 5 positions around the face to represent vowels. Associating one handshape with one position will cue a whole syllable.
Why is there a Deaf culture? What does it entail?
Now, it gets complicated. And fascinating. I won't dwell too much on the subject because I'm basically a baby Deaf gal and I'm not qualified. But my ask box is open if you would like to chime in.
From the moment people share a language and a history, they'll also share a set of experiences and issues, values, good practices, and basically a mindset. So naturally, with sign language being a real language, taught from generation to generation - as well as surviving a century-long ban from schools in France - a whole culture emerged.
And I should even say cultures… every country has its own Deaf culture because it's also tightly connected to the host country's culture and local sign language. The word "house" isn't signed the same way in French and Chinese sign language for instance because architecture isn't the same.
Deaf culture is very militant as Deaf people don't think of hearing loss as a disability. Deaf people typically don't speak of hearing loss but of "Deaf gain". I mentioned that there is systemic pressure to make hearing loss disappear. For instance by implanting deaf children at a young age to teach them to speak and integrate them into the hearing community. Deaf people see this as the death of their culture and language. For many people with hearing loss, Deaf culture means comfort, pride and community. From the Deaf perspective, hearing societies often fail to understand Deaf people's abilities and culture. From this point of view, the lack of communication stems as much from the inability of most hearing people to use sign language as from deaf people's inability to hear.
Deaf culture is however an open culture, you can join at any point in your life, you don't need to be born into it. It usually starts when you learn sign language and get acquainted to other d/Deaf people. A lot of people with late hearing loss are unaware such a culture exists and it often results in a lower self-esteem, as they don't find support in the hearing world. Some hearing people, especially relatives who learned sign language or interpreters, may be considered part of the Deaf community.
I'm not enough of an expert to write a full paragraph on the subject, but just know that just as in any culture, the arts are also very important in Deaf culture, especially visual performances, like drama and sign singing (chansigne in French).

Where can you learn sign language in France? Where can you practice? Do you have any online resources?
First things first, sign language isn't universal. There are almost as many sign languages as countries in the world, though some are more closely related than others. American Sign Language (ASL) and British Sign Language (BSL) differ widely as they use different basic signing systems. There is something called International Sign Language (ISL), but it is only used when Deaf people from different cultures meet, for instance during international competitions.
Regarding French Sign Language (FSL), there are a lot of organizations and associations which offer lessons such as l'Académie de la Langue des Signes Française, International Visual Theatre, 2LPE, Visuel-LSF, etc. Unfortunately, many (especially cultural ones) are located in Paris, and more sparsely in big cities like Lyon or Toulouse. If you're lucky, some high schools offer sign language as a third language, which allows you to gain extra credit for your baccalauréat.
Learning French Sign Language is very expensive, around 220€ for 30 hours. Financing for sign language can be hard to find and there are hardly any discounts or reimbursement schemes for people with hearing loss. Obviously, you may learn sign language through less expensive options, such as meetings at Cafés signes, which are small gatherings in a bar and welcome hearing people as well as deaf and HOH people. It's best to know at least the basics and to avoid Signed French (sign language has a different syntax) and several Cafés are limited to people with a certain level, but several associations like La Parole aux Sourds welcome everyone.
Some libraries in Paris have a dedicated section for sign language and Deaf culture such as Bibliothèque Louise Walser-Gaillard (next to the International Visual Theatre), Bibliothèque Saint-Eloi, La Canopée, Bibliothèque André Malraux and Bibliothèque Fessart. Books are great and may include a lot of signs, but they don't render movements very well.
There are plenty of online resources. I personally use Elix when I need to find a sign. It's a participative dictionary and still a work in progress, so all signs aren't referenced. What I find nice is that they provide variations of the same word. For instance, the word "mom" has a lot of “synonym” signs, depending on the area you live in.
Youtubers are an option BUT make sure you choose channels managed by a member of the Deaf community, a certified teacher and not a student learning sign language. It's important. I like Mélanie Lemaistre who has 2 channels. Her main one is MélanieDeaf, where she raises awareness on culture and sign language. On Signe2mains, she teaches a lot of vocabulary. You may also be interested in Lucas Wild's videos in which he signs and provides captions. It's good for training. And finally, I discovered this channel when typing this post and found it really, really good. It teaches idiomatic expressions in FSL.
But the most important thing when you want to learn sign language, like any living language, is to meet actual deaf people to practice. You have to learn to express yourself with your body and your face, and it's not easy for everyone. When you take lessons, the first hours are usually dedicated to mimes. Sign language is different than mimes as the frame is limited to your upper body but you still need to work on it at first. Signing people typically don't appreciate when you finger spell a word to learn its sign, it's more respectful to mime it.
Where can I meet Deaf people and learn more about their culture?
You may learn more about Deaf history and culture during Open Door Days, when organizations like the INJS (Institut National des Jeunes Sourds, one of the oldest schools for the d/Deaf in Paris) organize guided tours.
In Paris, there is also an amazing center called International Visual Theatre, which is dedicated to visual arts. I take sign language lessons there (they're excellent) and often go and see their plays, which are in both French and FSL.
Every two years, the city of Reims holds a huge international Deaf culture festival named Clin d'Oeil. Thousands of d/Deaf people come from all around the world to meet during 3-4 days (usually in July). It’s dedicated to sign language and visual arts: drama, dance, sign singing (chansigne in French), cinema, mimes, street performances… The next edition will be held in 2021.
Deaf people also organize gatherings, for instance during the World Day of the Deaf, every year in September, when we march for our rights.
Regarding the media, France Télévisions has a program called "L'Œil et la Main". It's in FSL and subtitled in French. They address current news (#MeToo, Youtubers, gene therapy, etc.) from a deaf perspective. There's also a news website entirely designed for signing deaf people called Media'Pi. In FSL, "Pi" means everything that's typical or related to a situation or a person and there's no exact translation in French. If you understand French, this subtitled video explains what "Pi" means.
Do you have any movies, series, books, etc. to recommend?
To be perfectly honest, I have yet to find entertainment media about deafness and hearing loss that I like. But I do like when movies or series include deaf characters and d/Deaf actors into their ensemble. I haven't watched Switched at Birth, but I liked Sean Berdy in what I've seen of The Society so far. There's another young deaf actress, Millicent Simmonds, who was recently in 2 movies: A Quiet Place and Wonderstruck (I'd like to see the latter one). Marvel's The Eternals will also feature the first deaf superhero, played by Lauren Ridloff. But none of them really address deafness and Deaf culture. Which is also totally valid, it's nice to have characters where aren't defined by their disability. It's just that yes, sign language is nice, aesthetic through the roof, but what about the culture?
I didn't like Children of a Lesser God (a movie based on a play), which is supposedly one of the most famous representation of deaf people in movies. I think it didn't age well. But it made Marlee Matlin famous and allowed her to talk about Deaf issues and to give the community visibility. It's a must-see, basically, but you might not enjoy it.
In France, we have hardly any representation in the mainstream media, which is why I have high hopes about Skam France and I hope I can soon add it to the list. I however recommend this documentary: J'avancerai vers toi avec les yeux d'un sourd, which the director dedicated to the friend who had taught her sign language and who sadly passed. I don't think this was distributed in English but I could be wrong! There's also Marie Heurtin, a movie about a young deafblind girl who is sent to a convent, where Sister Sainte-Marguerite decides to take care of her.
The only popular movie with deaf characters that came out in theatres recently in our country was La Famille Bélier. It's quite infamous within the Deaf community as hearing actors who were specially trained in FSL for the movie played two out of the three main deaf characters. We're used to having our roles played by hearing actors, the same happened to the recent theatre production of Children of a Lesser God where two hearies played the main characters.
I know that many people think cinema and drama are all about the performance and that it's ok when hearing people play minority roles. It's not. Have you ever seen a deaf actor play a hearing character?
I'm short on fiction works about hearing loss and Deaf culture though; feel free to chime in if you have recs. In France, Emmanuel Laborit, a French Deaf icon who won a Molière in the 90s for her performance as Sarah Norman (the main character of Children of a Lesser God), wrote an autobiography, le Cri de la Mouette where she addresses her childhood memories and the shaping of her identity though sign language.
What would help d/Deaf people?
First and foremost: visibility! Let d/Deaf people speak for themselves, demand from production companies that they cast deaf people for deaf roles. If you are not seen, you are not known. Invisibility plays a big role in the negative perception hearing people have of hearing loss. I've seen people here saying that becoming deaf was their worst fear in the world. How sad.
Many people with hearing loss will spend their whole life with it. We have a long road before us and we don't want to fight it through. Give us positive representation, people to admire and to support. Give us good stories. Giving us visibility will also shine a light on our issues and our needs regarding accessibility. Keep that in mind and be proactive. You don't realize how tiring it is, when you are deaf, to organize your own accessibility at an event because nobody thought about it. Equal opportunities don't mean you have to treat us like you would treat abled people and just let us be (you know we are already aware we have the right to exist?). It means giving us the tools we need to thrive because we start with a pretty big impediment.
Don't let deaf people bear the brunt of the lack of communication, especially if they choose not to talk. You wouldn't do it with a foreigner. Use voice recognition or type your sentence on your smartphone, use mimes, but just try. A conversation goes both ways and we are the disabled ones, not you.

There are so many things I could be writing about right now. 
I’ve chosen to stick with one of the things I know best for this post.

A handful of pictures popped up in my Timehop from 12 whole years ago that gave me pause the other day.
Sometimes when I see older pictures of myself I am overwhelmed by how different I look now... in a bad way. I see myself in those pictures as thin and beautiful and I see myself now as a sausage monster stuffed into bike shorts. 
The more I sit with and work on my body image issues, the more I have noticed healthier thinking habits developing. Let me be clear, this has been an incredibly slow process. But seeing those changes is something I am really encouraged by... and it makes the every day body image fight have some measurable value. 
I can say with confidence that, at 200 pounds, my body image is currently the best it has ever been.
That has nothing to do with the specific number on the scale and everything to do with working really hard over a lot of years to understand that neither “beauty” or “health” are inherently defined as “thin.” A fundamental pillar of that understanding is that you cannot separate mental health from the concept of general health. 
Mainstream culture does this. 
Mainstream culture wants you to believe that it’s your weight or your BMI that determines whether or not you are healthy.
That is bullshit. 
Here are the pictures of me from 12 years ago. We were moving my high school boyfriend into his freshman dorm for his first year of college. I don’t think any of the people in these photos will mind me sharing them in the context of this blog post.

I am *THIN* in these pictures.
I am 17.
Apart from a very clear warning sign that I was already developing horrible posture, I noticed a couple of things right away about these pictures when I was looking at them the other day.
My shirt is a size small or extra small. 
I am holding my arm across my stomach in the fourth picture because I do not think I am *thin enough* to be wearing that shirt. 
I may have been a thin 17-year-old. But I was not healthy.
I was physically fit.
I played soccer for three out of four seasons of the year.
But I was not healthy.
I know I wasn’t healthy because I was about to embark on my senior year of high school during which I would, at times, only allow myself one and a half meals per day. Sometimes that one meal would be pasta. Other times that one meal would be a bag of dark chocolate peanut M&Ms and a Mountain Dew. Other times it would be a gallon of strawberries. Other times it would be a family pack of Twizzlers.
My body in those pictures might look healthy. 
But appearance is not an appropriate indicator of health. 
The two times in my adult life that I have been the thinnest have also been the times in my life where I have struggled the most with body image and disordered eating. 
After I escaped the abusive relationship of my freshman year of college, I gained around 20 pounds.
The following summer, I exercised for an hour every day and ate only pickles and Greek yogurt (separately, of course, don’t be gross). 
I lost 30 pounds.
That was also not healthy.
Fast forward a handful of years to 2015. 
I start an anti-depressant. 
Over the course of the next two years I gain around 50 pounds.
Today, in the spirit of full disclosure (and because the numbers don’t mean shit), my weight fluctuates between 190 and 200. 
I am obese.
A mathematical algorithm used to determine BMI has labeled me “obese.” 
My clothing sizes vary day to day thanks to IBS-related bloating but I’m somewhere around a 14-16. 
Do you know the cut-off for plus sizes? 
It’s 14. 
So, I am an obese, plus-sized woman.
The numbers aren’t very polite, are they?
Within the last two years I was diagnosed with severe iron-deficiency anemia.
I committed to correcting that with a number of lifestyle changes including taking supplements and adding iron-heavy foods to my diet. I took Vitamin C to boost my absorption. On days I took the supplement I had no coffee, no tea, no dairy, and no acid-reducer meds. In 6 very committed months, I resolved my iron issues, for the most part. We have since learned that the daily stomach medicine I take may be affecting my iron absorption so, although I am no longer taking supplements, I am taking a daily vitamin to help maintain a healthy level of iron. 
That story is about health.
I had a health issue and I developed a strategy to resolve the issue, being sure to consider my mental health as well. 
I have worked really, really hard to consider my HEALTH instead of my WEIGHT.
This obese, plus-sized woman exercises for around an hour every day. She does not drink alcohol or soda. She is aware of what she eats and is careful to eat when she’s hungry and stop when she’s full. 
One of the ways I know my thinking is healthier is that when I look at bathing suits on Target’s website, I have started to consider their plus-sized models “normal.” 
The average size of an American woman based on the most recent data is between sizes 18 and 20. 
I spent over twenty years unable to see an average-sized woman as beautiful.
Even though the clothing industry has labeled me “plus-sized,” if anything, I am “slightly less than average-sized.”

Coming off testosterone
I stopped taking testosterone on day 235, May 19th 2019 - 6 days ago, and after just under 8 months of low-dose testosterone. I had 1 pump of Tostran gel per day, which the medication leaflet says is 10mg of testosterone.

WHY DID I START TAKING LOW-DOSE TESTOSTERONE?
I’m nonbinary, and my goal was always to be androgynous. I started taking testosterone to deepen my voice, mainly, but the idea of a more masculine fat distribution on my body was appealing because I also have dysphoria about my hips and slim shoulders. “Passing” as nonbinary isn’t really a thing because most people don’t know that there are more than two genders, so the best I could really hope to achieve realistically was to confuse as many people about my gender as possible. 😈
Changes I wanted:
Deeper voice
Genital changes
Less feminine body fat distribution
Changes I didn’t want:
Facial hair
Hairline changes (but if it happens a little that’s okay)
Loss of hair on head
Body hair
A couple of years ago I had one session of voice therapy (NHS), which was fun and very interesting, but it taught me a couple of things. One was that finding my lower pitches and getting that low resonance by finding my chest voice was definitely helpful and relieved my voice dysphoria. The other was that there was no way I was going to be able to keep up with multiple voice exercises per day to keep it that way. I pretty much decided that day that testosterone was the most sensible option, and even though it was scary I would just have to find a way to deal with the negative effects.
My voice dysphoria was sort of disconnected from my idea of my gender. All I really knew was that my voice needed to be a lower pitch and more resonant. I felt that dysphoria especially when I was talking excitedly or when I was singing. But I also knew that I didn’t want people to mistake me for a man either, and since it’s a very long slow process and quite unpredictable I knew that I would need to be careful to keep track of things to make sure I didn’t start giving myself a new kind of gender dysphoria in the masculine direction!
It was another year or two before the stars aligned and I actually managed to get the can of Tostran into my hands (NHS).

HOW DID IT GO?
As I mentioned above, I was on a low dose, one pump of Tostran per day. One can lasted about 107 days. The endocrine nurse said I could reduce the dose to half if I squirted the gel onto my leg/stomach as usual and then wiped half the gel-dribble off with tissue and threw it in the bin before I rubbed it in, but I never did that. The two-month blood test put my levels at 9.7 nmol or something like that. (Female range is 3 or below, and male range is 10 or above, so I was very nearly almost at the male range.)
The first month I got that scratchy throat feeling, which I now know happens when my voice is deepening, and I noticed my voice getting a little lower. Also I was SO GREASY OH MY GOD, I had to shower twice as much, I had to get some high-powered anti-perspirant deodorant. It didn’t make me spotty, it was just disgusting! But it felt okay because I could tell it was affecting my voice. I also noticed that I smelled different, and that my bedroom acquired that delightful(??) teenage boy smell! Ew. This kicked in properly in under a week, and trailed off around month two, though I still needed to take more showers and wear hardcore deodorant and I was still more greasy for the entire time I was on T.
On day 26 I noticed that my... [flails around wildly for non-dysphoric word] pudendum had gotten a little bigger, and I regret not getting “before T” photos. The growth was fast and a lot more noticeable than I had expected, and it was extremely emotionally intense, because I hadn’t realised but I’d been sort of suppressing and dissociating from genital dysphoria for my whole life. I knew I had genital dysphoria, but not how much or how it was affecting me. I was completely unprepared, and it was overwhelming. (I am very lucky that I was able to access fortnightly gender-specialist talking therapy at Charing Cross GIC during this time.) A few weeks later I started looking into genital surgery options. I think my genitals haven’t changed much in the last 2 months, so I guess it took about 5-6 months to get to where they are now.

In the second graph, the thing I notice right away is that my highest pitch now is lower than my lowest pitch pre-T.
My first month was rapid pitch drop, and then there were a couple of months of wibbling around on a plateau, and then after that things kicked off again, I had a few scattered days of scratchy throat and things started meandering downward. A common pattern was two to four weeks of wild fluctuation and no drop, followed by a sudden drop over a day or two. Sometimes my brain took a while to adjust to a drop in pitch, and I would tire myself out speaking with a higher pitch than my vocal chords really wanted to do.
In the last couple of months I got a lot worse at making the voice recordings, which in hindsight might be because I was less enamoured with being on testosterone, and it was maybe a sign that my testosterone advantages were sort of wrapping up and it was nearly time to stop.
No one who sees me regularly said unprompted that they could hear a difference in my voice. When I asked people if my voice sounded different, they said “ehhh, maybe kinda, yeah?” I learned that testosterone doesn’t make your voice sound different! It makes it feel different, to yourself and to other people. My therapist, who I see (approximately) fortnightly, said she couldn’t tell the difference at all, and when I played her my day 1 voice recording to compare it to my current voice (6 months or so) she was like 😮.
Strangers are still mostly gendering me female, as far as I can tell, but my appearance and my voice means that strangers gender me male a little more often than pre-T. I also get “sir, I mean ma’am, gosh I’m so sorry...” more often. It makes me uncomfortable to make people uncomfortable, so I just usually say “it’s okay, I’m nonbinary so I’m kind of both??”, which rarely makes them less flustered or less confused. I had some cards made years ago that have the nonbinary flag on one side and the definition of nonbinary on the other, which I should just start handing out whenever this happens!
My testosterone dose was too low to stop my menstrual cycle, but it did seem to mellow out the highs and lows. My PMS and menstrual symptoms are generally a nightmare even though I have no uterus to bleed from, so that was a nice relief. It was very strange to experience PMS and boy puberty simultaneously.
I kept records of my specific dysphoria feelings from about 3.5 months, thanks to prompting from my gender-specialist therapist at Charing Cross GIC in London:

The ideal outcome, which I was looking out for, was all of the lines meandering towards that horizontal line in the middle. That would mean the feminine dysphoria was reducing and the masculine dysphoria was reducing.
I am pretty lucky, in that most of the changes I wanted from testosterone are permanent and the changes I didn’t want are temporary or reversible. I made some lines on the graph more bold because those were the ones that I wanted to keep an eye on. Here are my thoughts on this graph:
Facial hair was impossible to score because it gave me both masculine and feminine dysphoria. Feminine because those dark fine hairs on my upper lip are most often seen on cis women, and masculine because they were caused by testosterone and male puberty.
When my pudendum started growing it also gave mixed signals. It was good because it was getting bigger, but as it grew it highlighted that there was a lot going on down there that made me feel feminine dysphoria (mainly labia around my pudendum, and sometimes that it was still relatively small).
I am not a very hairy person. I thought I wouldn’t like having more body hair, and for a few months when I scored my dysphoria I focused on specific patches of new hair and thought about whether I was dysphoric and thought that I wasn’t. After a few months I realised I was being too short-sighted. Yes, I can totally deal with these new fine hairs on my thighs. Yes, this darker and thicker hair on my shins is okay. Sure, these hairs around my belly button are no big thing. But when I took a step back and looked at the bigger picture, and saw the hair as a pattern, my whole brain recoiled. Yes, I am definitely dysphoric about masculine patterns of body hair on myself. :S
There was a trend of all types of dysphoria getting less over time - until month 7, when they all started getting worse except voice. It took a while to be sure it was a trend, but looking back on it, that was definitely a thing!
The ideal outcome would be all of those lines converging on the middle horizontal axis, and it hasn’t happened. There are no flawless solutions for nonbinary people at the moment. As things stand and with what’s available to me right now, I will probably always have gender dysphoria. But I’m pretty glad that I’ve done what I can and I will keep doing what I can, because it is all helping.
Unrelated to any gender stuff, I have put on weight and gone up a size or two in trousers and one size in t-shirts. It could just be that I’m in my 30s and my metabolism is slowing, which is what I assumed it was at first, but the weight has come on in quite a short space of time, so that made me think about what I’ve been eating and why. I noticed that I was craving carbs and sugar, eating it, and then feeling unsatisfied afterwards. The type of hunger I’ve been feeling lately feels the same as the type of hunger I had when I had Implanon, a three-year birth control implant. When I had that removed my appetite went back to normal. So this putting on weight feels a lot like a hormonal thing. I have mixed feelings about it! I strongly dislike my clothes feeling wrong and having to buy new clothes, but also bits of me are pleasingly wibbly and it’s fun to be able to rest my cup of tea on my stomach when I slouch. :D
Also unrelated to gender stuff, even on a low dose many of my EDS symptoms were noticeably reduced: fatigue, loose joints, joint pain, IBS, dysautonomia. (Joint pain and fatigue were still present, but not as bad after exertion.) I also noticed that the really vivid dreams and very emotional days that come with PMS mostly disappeared, which I was sad about.

HOW DID I KNOW TO STOP?
People gendering me in different ways has increased lately. Like, a few weeks ago me and @watchkeyphone were trundling about town, and one charity/religion street-hawker person asked if we were sisters, and then a hundred yards later their colleague called us “lads” or something.
A week or so ago, I realised that my voice was sounding and feeling resonant in my chest pretty much all the time and that felt pretty nice, but I was still scoring my voice as sounding feminine, and I wondered if that might be because the changes are so gradual that I just changed my idea of what feminine sounds like. I noticed that I was more comfortable recording my voice to put online for various reasons.
I don’t live in a culture where people go around saying “excuse me sir” or “hello madam”, we don’t routinely gender each other in a formal way. But also, either I don’t have enough casual contact with strangers to notice gendered familiar words like “mate” or “love”, or strangers just avoid using those kinds of words with me because I’m hella queer-looking. So I basically realised that I have no idea how strangers see me or hear my voice at all.
So, in approximately this order:
I put a recording of my voice on the internet and asked strangers to gender my voice. Most of them said androgynous, leaning a little masculine. One person said I sounded like Q, a computer-generated intentionally genderless voice!
I went to a queer social group, and when it came up in conversation naturally I complained about how hard it is to know when to stop testosterone because I can’t tell how my body and voice are gendered by other people, and a lot of people I see regularly still see me the same way as they saw me pre-T. A couple of people said, “actually, in the past couple of months I have really noticed the effects...” So that was reassuring!
I decided to notice how my pitch works in different situations compared to pre-T. After the queer social group we went down to the river and some people swam, and when they got into the water and it was very cold, some of us cheered supportively - and I tried to woo like the “woo girls”, and my voice just came out at a dude pitch instead...! I apparently can’t be high-pitched at high volumes any more.
I watched a video about gender and voice by someone I’ve met in person. When I met them I noticed that their voice was pretty androgynous, and I enjoyed it and thought about how nice it must be to have a voice that can’t be easily gendered by strangers. When I watched this video last week, in which their voice was exactly the same, I noticed that they sounded like me. That was the moment that I realised my voice was done!
I spoke to my PA about it. I played my pre-T voice to her and her face was A Picture, she could not even. She then said that she has quite a feminine voice, and she suggested we each make a recording of our voices reading a paragraph of the book on the table, and then compare to her partner’s voice in a recording. My voice sounded more like her male partner’s voice than like hers.
I can now speak with a comically low deep resonant voice if I want to, and I can also speak with a high cutesy voice in order to address my cat, provided I am warmed up a bit.

HOW DID STOPPING GO?
On the day that I posted the voice file online, when strangers started saying I sounded masculine, I was honestly pretty surprised. In my head strangers in person were still mostly gendering me female, but when I really thought about it people hadn’t actually been gendering me much at all. I think I had been assuming strangers were hearing a woman’s voice because the change had been so gradual that I hadn’t had a moment where I could “update” my own gendering of my voice. I didn’t wake up one day and go “wow I sound like a dude” or whatever, so there was just nothing to update.
So, as soon as there was an indication that I might sound like a man soon, my gut said “NOPE” and I worried a little bit about going too far in the masc direction. If I keep taking T then I will sound more masculine and I might regret it, but if I stop and find that my voice dysphoria could be relieved with a little more testosterone I can start again in a few months, right? So I decided to not take it that day unless I learned/felt something that indicated I should put the gel on, and... I didn’t.
The first couple of days were pretty uneventful. Around day 4 I started to feel really run-down and chronic joint pain from EDS was flaring up, so I cancelled near-future plans. Yesterday was like the worst of my (uterusless) periods, I was in a lot of abdominal and lower back pain and then last night I slept for 11 hours... And today, also typical of my uterusless periods, it feels like a storm has blown over and I feel like a new human.
So what I’ve concluded is, coming off T triggered a very, very bad period.
Also, the past couple of days I have once again been SO GREASY, and I got spots yesterday, which is unusual in itself, but these are striking because they are WEIRDLY HUGE?? One of them is on my jaw and has caused a very noticeable swelling, so I’ve named it Balthazar.

WHAT NEXT?
It’s only been a week, so I’m assuming more weirdness is coming, but it’s all going to be ovary-hormone-related silliness so I’m pretty much used to it and I’m not too worried.
I am sad that I will probably slip back towards feminine body fat distribution. I will probably lose some of the weight that testosterone brought with it, but the remaining weight will probably end up on my hips again. I am really not looking forward to dissociating my hips again but I don’t know what I can do about it except have liposuction every few years?! (I will not do that.)
I am also sad that the bad bits of my menstrual cycle will go back to Full Force, and that my EDS symptoms will worsen again.
I am very much looking forward to my body hair getting finer and lighter, and maybe my upper lip hair will fade a bit too. If not I will probably have to get it painfully removed.
I want a metoidioplasty. Unlike many trans guys, I want no testicular prostheses, no vaginectomy, no phalloplasty, no new urethra. The clinicians at Charing Cross are aware that I want to have a metoidioplasty, because I included it in a letter when I wrote to the endo about a blood test, to make sure my surgery needs are documented in my medical records from the earliest date possible. I did that because they have minimum-time requirements for a bunch of diagnoses and referrals (like, two appointments before hormone treatment recommendation), which you can sometimes get around by providing reasonable counter-arguments. One of their requirements for referring for genital surgery is being on testosterone for at least a year where clinically indicated, so since testosterone has done its job now that means I’m not clinically indicated for testosterone any more, and I’m hoping that being on T for only 8 months shouldn’t be an issue. I also found a blog by a trans guy who had a simple metoidioplasty like the one I want, and his blog posts are really helpful and informative and have good quality non-porn photos (so rare omg), and his junk and my junk are extremely similar in size and appearance. (Here are his before and after pics.) So, fingers crossed the docs will consider that I have been on T for long enough and my pudendum is developed enough that surgery is an option.
I’ve written to the endocrinologist to say that I’ve stopped putting on the Tostran gel, and to ask if I still need to have those blood tests I’m meant to be having. I’m due to see him on 1st July anyway, a little over a month from now, and no doubt I will have saved up a list of questions for him!

Obese patients receive worse care solely based on their weight
”You must lose weight, a doctor told Sarah Bramblette, advising a 1,200-calorie-a-day diet. But Ms. Bramblette had a basic question: How much do I weigh?
The doctor’s scale went up to 350 pounds, and she was heavier than that. If she did not know the number, how would she know if the diet was working?
The doctor had no answer. So Ms. Bramblette, 39, who lived in Ohio at the time, resorted to a solution that made her burn with shame. She drove to a nearby junkyard that had a scale that could weigh her. She was 502 pounds.
One in three Americans is obese, a rate that has been steadily growing for more than two decades, but the health care system — in its attitudes, equipment and common practices — is ill prepared, and its practitioners are often unwilling, to treat the rising population of fat patients.

The difficulties range from scales and scanners, like M.R.I. machines that are not built big enough for very heavy people, to surgeons who categorically refuse to give knee or hip replacements to the obese, to drug doses that have not been calibrated for obese patients. The situation is particularly thorny for the more than 15 million Americans who have extreme obesity — a body mass index of 40 or higher — and face a wide range of health concerns.
Part of the problem, both patients and doctors say, is a reluctance to look beyond a fat person’s weight. Patty Nece, 58, of Alexandria, Va., went to an orthopedist because her hip was aching. She had lost nearly 70 pounds and, although she still had a way to go, was feeling good about herself. Until she saw the doctor.
“He came to the door of the exam room, and I started to tell him my symptoms,” Ms. Nece said. “He said: ‘Let me cut to the chase. You need to lose weight.’”
The doctor, she said, never examined her. But he made a diagnosis, “obesity pain,” and relayed it to her internist. In fact, she later learned, she had progressive scoliosis, a condition not caused by obesity.
Dr. Louis J. Aronne, an obesity specialist at Weill Cornell Medicine, helped found the American Board of Obesity Medicine to address this sort of issue. The goal is to help doctors learn how to treat obesity and serve as a resource for patients seeking doctors who can look past their weight when they have a medical problem.
Dr. Aronne says patients recount stories like Ms. Nece’s to him all the time.
“Our patients say: ‘Nobody has ever treated me like I have a serious problem. They blow it off and tell me to go to Weight Watchers,’” Dr. Aronne said.
“Physicians need better education, and they need a different attitude toward people who have obesity,” he said. “They need to recognize that this is a disease like diabetes or any other disease they are treating people for.”
The issues facing obese people follow them through the medical system, starting with the physical exam.
Research has shown that doctors may spend less time with obese patients and fail to refer them for diagnostic tests. One study asked 122 primary care doctors affiliated with one of three hospitals within the Texas Medical Center in Houston about their attitudes toward obese patients. The doctors “reported that seeing patients was a greater waste of their time the heavier that they were, that physicians would like their jobs less as their patients increased in size, that heavier patients were viewed to be more annoying, and that physicians felt less patience the heavier the patient was,” the researchers wrote.
Other times, doctors may be unwittingly influenced by unfounded assumptions, attributing symptoms like shortness of breath to the person’s weight without investigating other likely causes.
That happened to a patient who eventually went to see Dr. Scott Kahan, an obesity specialist at Georgetown University. The patient, a 46-year-old woman, suddenly found it almost impossible to walk from her bedroom to her kitchen. Those few steps left her gasping for breath. Frightened, she went to a local urgent care center, where the doctor said she had a lot of weight pressing on her lungs. The only thing wrong with her, the doctor said, was that she was fat.
“I started to cry,” said the woman, who asked not to be named to protect her privacy. “I said: ‘I don’t have a sudden weight pressing on my lungs. I’m really scared. I’m not able to breathe.’”
“That’s the problem with obesity,” she said the doctor told her. “Have you ever considered going on a diet?”
It turned out that the woman had several small blood clots in her lungs, a life-threatening condition, Dr. Kahan said.
For many, the next step in a diagnosis involves a scan, like a CT or M.R.I. But many extremely heavy people cannot fit in the scanners, which, depending on the model, typically have weight limits of 350 to 450 pounds.
Scanners that can handle very heavy people are manufactured, but one national survey found that at least 90 percent of emergency rooms did not have them. Even four in five community hospitals that were deemed bariatric surgery centers of excellence lacked scanners that could handle very heavy people. Yet CT or M.R.I. imaging is needed to evaluate patients with a variety of ailments, including trauma, acute abdominal pain, lung blood clots and strokes.
When an obese patient cannot fit in a scanner, doctors may just give up. Some use X-rays to scan, hoping for the best. Others resort to more extreme measures. Dr. Kahan said another doctor had sent one of his patients to a zoo for a scan. She was so humiliated that she declined requests for an interview.
Problems do not end with a diagnosis. With treatments, uncertainties continue to abound.
In cancer, for example, obese patients tend to have worse outcomes and a higher risk of death — a difference that holds for every type of cancer.
The disease of obesity might exacerbate cancer, said Dr. Clifford Hudis, the chief executive officer of the American Society of Clinical Oncology.
But, he added, another reason for poor outcomes in obese cancer patients is almost certainly that medical care is compromised. Drug doses are usually based on standard body sizes or surface areas. The definition of a standard size, Dr. Hudis said, is often based on data involving people from decades ago, when the average person was thinner.
For fat people, that might lead to underdosing for some drugs, but it is hard to know without studying specific drug effects in heavier people, and such studies are generally not done. Without that data, if someone does not respond to a cancer drug, it is impossible to know whether the dose was wrong or the patient’s tumor was just resisting the drug.
One of the most frequent medical problems in obese patients is arthritis of the hip or knee. It is so common, in fact, that most patients arriving at orthopedists’ offices in agonizing pain from hip or knee arthritis are obese. But many orthopedists will not offer surgery unless the patients first lose weight, said Dr. Adolph J. Yates Jr., an orthopedics professor at the University of Pittsburgh School of Medicine.
“There are offices that will screen by phone,” Dr. Yates said. “They will ask for weight and height and tell patients before they see them that they can’t help them.”
But how well grounded are those weight limits?
“There is a perception among some surgeons that it is more difficult, and certainly some felt it was an added risk,” to operate on very obese people, Dr. Yates said. He was a member of a committee that reviewed the risks and benefits of joint replacement in obese patients for the American Association of Hip and Knee Surgeons. The group concluded that heavy patients should first be counseled to lose weight because a lower weight reduces stress on the joints and can alleviate pain without surgery.
But there should not be blanket refusals to operate on fat people, the committee wrote. Those with a body mass index over 40 — like a 5-foot-5-inch woman weighing 250 pounds or a 6-foot man weighing 300 — and who cannot lose weight should be informed that their risks are greater, but they should not be categorically dismissed, the group concluded.
Dr. Yates said he had successfully operated on people with body mass indexes as high as 45. What is behind the refusals to operate, he said, is that doctors and hospitals have become risk-averse because they fear their ratings will fall if too many patients have complications.
A lower score can mean reductions in reimbursements by Medicare. Poor results can also lead to penalties for hospitals and, eventually, doctors.
A recent survey of more than 700 hip and knee surgeons confirmed Dr. Yates’s impressions. Sixty-two percent said they used body mass index scores as cutoffs for requiring weight loss before offering surgery. But there was no consistency in the figures they picked.
“The numbers were all over the map,” Dr. Yates said. And 42 percent who picked a body mass index cutoff said they had done so because they were worried about their performance score or that of their hospital.
“It’s very common to pick an arbitrary B.M.I. number and say, ‘That is the number we won’t go above,’” Dr. Yates said. Yet a person with an index of, say, 41 might be healthy and active, he said, but in terrible pain from arthritis. A knee replacement could be life transforming.
“It’s a zero-sum game, with everyone trying to have the lowest-risk patient,” Dr. Yates said. “Patients who may be at a marginally higher risk may be treated as a class instead of individuals. That is the definition of discrimination.”
Surgery involves anesthesia, of course, giving rise to another issue.
There are no requirements for drug makers to figure out appropriate doses for obese patients. Only a few medical experts, like Dr. Hendrikus Lemmens, a professor of anesthesiology at Stanford University, have tried to provide answers.
His group looked at several drugs: propofol, which puts people to sleep before they get general anesthesia; succinylcholine, used to relax muscles in the windpipe when a breathing tube must be inserted; and anesthetic gases.
Propofol doses, Dr. Lemmens found, should be based on lean body weight — the weight of the body minus its fat. Using total body weight, as is routine for normal-weight people, would result in an overdose for obese patients, he said. But succinylcholine doses should be based on total body weight, he determined, and the dosing of anesthetic gases is not significantly affected by obesity.
As for regional anesthetics, he said, “There are very few data, but they probably should be dosed according to lean body weight.”
“Bad outcomes because of inappropriate dosing do occur,” said Dr. Lemmens, who added that 20 to 30 percent of all obese patients in intensive care after surgery were there because of anesthetic complications. Given the uncertainties about anesthetic doses for the obese, Dr. Lemmens said, he suspects that a significant number of them had inappropriate dosing.
Yet for many fat people, the questions about appropriate medical care are beside the point because they stay away from doctors.
“I have avoided going to a doctor at all,” said Sarai Walker, the author of “Dietland,” a novel. “That is very common with fat people. No matter what the problem is, the doctor will blame it on fat and will tell you to lose weight.”
